Student Registration

Registration Please contact the District Registrar for all new registrations. Children are eligible to begin Kindergarten if they turn 5 years of age on or before December 1st of that year. At the time of registration, parents/ guardians must provide the child’s birth certificate or other official proof of age, proof of immunizations and proof of residency in Island Trees School District. The Registrar’s office is open over the summer.

 

MINIMUM REQUIREMENT FOR SCHOOL ATTENDANCE:

• DTaP, DTP: 4-5 doses required (final dose after age 4)

• Tdap: required for students grades 6-12, must be received by age 11

• POLIO (IPV/OPV): 3-4 doses required( final dose after 4yrs)

• MMR: 2 doses required for all grades

• HEPATITIS B: 3 dose series required

• CHICKEN POX: 2 doses required for Kindergarten, all grades

• Meningitis: 1 dose required in 7th and 12th grade. (new for 2016)
